Medical Card stapled / inserted to the passport

Copy of Marriage Certificate* for Couples. |

Copy of Birth Certificate* for the children (*6 & *7 are required if Last Names on the passport are different) |

New Muslims who have not changed their names on their passport are required to submit a statement in writing from their local Islamic center validating their conversion. |

A pre-paid return airway bill a carrier of your choice must be included for securely returning your processed documents to you. |
